Fairfax Police Academy Magnet - Enrollment for New Students Fall 2020 All students accepted by June 10, 2020, will receive an enrollment packet in the mail. After June 10th, you will need to download the forms, print them out and mail or drop them off to Fairfax High School. Fairfax High School will be open form 8:00 - 11:00 daily June 11th - June 11- June 30 FAIRFAX HIGH SCHOOL IS CLOSED THE ENTIRE MONTH OF JULY- DO NOT DROP OFF OR SEND FORMS DURING THIS TIME. Fairfax High School will reopen August 3rd and forms may be mailed or dropped off at that time. Please return by mail postmarked by June 24th or drop off at Fairfax High School by June 30th from 8:00 a.m - 11:00 a.m. Check off List of Required Forms to Return for Students New or Returning to LAUSD: Student Enrollment Form Uniform Measurement Form PAM Student Requirements PAM Short Answer/Essay Emergency Information Form Magnet office Emergency Information Form for the Attendance Office Emergency Information Form for the Nurse’s Office Publicity authorization (photo) and Release Student Housing Questionnaire The following are documents you must also include with your filled out enrollment forms: Most recent progress report card Copy of IEP, if your child has one (Special Education Eligible Students) Copy of 504 plan, if your child has one Copy of Proof of Residence that has at least one parent or guardian name and address on it. It may be one of the following: gas bill, electric bill, water bill, mortgage statement, renter’s agreement contract, or an official government piece of mail.
